Offer Terms & Conditions
Eligibility – new Earthmade patients only
This offer is available to new patients of Earthmade Clinic only. A “new patient” is someone who has not previously completed a consultation with, or been registered as an active patient of Earthmade.In these Terms and Conditions, unless otherwise indicated:
What the offer includes
The offer covers the consultation fee for one (1) standard initial telehealth consultation with an Earthmade clinician (usually $79).
It does not include:
- Any medication costs
- Pharmacy dispensing fees
- Script fees or follow-up consultation fees
One per person, non-transferable
The offer is limited to one free initial consultation per eligible patient. It is non-transferable, cannot be redeemed for cash or credit, and cannot be used in conjunction with any other offer.
Booking and consent requirements
To redeem the offer, you must:
- Book via the specific booking link provided in the email from Doctor Sync Technologies; and
- Complete Earthmade’s required intake and consent forms, including providing consent for [Doctorsync Technologies and Earthmade to access and transfer your relevant health records from the previous clinic on file (Dispensed Pty Ltd) for the purpose of assessing your eligibility and providing care.
Clinical eligibility and prescribing
All clinical decisions remain at the sole discretion of the treating clinician.
- Participation in this offer does not guarantee that a prescription for medicinal cannabis will be issued, changed or continued.
- You must meet any applicable clinical and regulatory eligibility criteria.
Availability and changes to the offer
Appointment times are subject to clinician availability.
Doctorsync Technologies and Earthmade reserve the right to amend, suspend or withdraw this offer at any time. Any changes will not affect free consultations that have already been booked and confirmed prior to the change.
